rethink Free consult:

15 minute virtual video call for you to share what brings you to rethinkAccessibility and ask any questions about our services or expertise.

rethink intake paperwork:

Complete virtual paperwork to collect a baseline including:

  • basic information about you and your living situation

  • questions about your current health and family history

  • identifying any current or future limitations in your living space, business or community mobility through our screening questions

In person visit:

We meet face to face to complete a comprehensive home/business/office/community center/etc. assessment during which we do a tour of the space, take pictures and measurements of the space to use as part of the rethinkreport.

INVOICE:

Invoice will be sent upon completion of in person visit. Once invoice has been paid you will be sent a pdf of your rethinkreport and a scheduling link for virtual follow up.

rethink FOLLOW UP:

Virtual video call to discuss findings of evaluation, and answer client questions about home modification or product recommendations.

Rethink COncierge:

  • additional to facilitation implementation of recommendations and provided continued feedback

  • collaboration with related professionals (i.e. local contractor, product manufacturers, etc.) to implement recommended modifications. We work with your preferred vendors or can connect you with our trusted professionals.

Rethink Functional mobility evaluation:

  • collaborating with your physician and a certified ATP from registered DME Vendor we can administer assessments needed to write a letter of medical necessity (LME) used for insurance authorization for device.

  • FME can be performed in person (with additional travel fee) or virtually via video call
